Your Personal Health Information
A free community based public education campaign

Every time you visit your doctor, hospital, or another healthcare provider, a record of your visit is made. This information is then compiled into what is known as your health record.

Your physical health record belongs to your healthcare provider, but the information in it belongs to you! Understanding what is in your health record helps you:

  • Make sure it’s correct and complete
     

  • Know what is being released when you authorize disclosure of information to others
     

  • Provide an accurate health history to all healthcare providers who treat you

Your health information is scattered across many different providers and facilities. Keeping your own complete, updated, and easily accessible health record means you can play a more active role in your healthcare. You wouldn’t write checks without keeping a check register. The same level of responsibility makes sense for your healthcare.

The American Health Information Management Association (AHIMA) is a national non-profit professional association, founded in 1928, dedicated to the effective management of personal health information needed to deliver quality healthcare to the public.

The New Jersey Health Information Management Association (NJHIMA) is a component state association of AHIMA. NJHIMA has over 1,000 members working to manage and protect your personal health information and medical records in hospitals, doctors’ offices, and other healthcare settings. Health information management professionals care for your health by caring for your health information. Their job is to make sure that all the medical information collected about you is complete, accurate, and protected, yet, at the same time, readily available for your healthcare providers when it’s needed. We now want to assist you in learning more about your health information.

Health information management (HIM) professionals are available to provide your group or organization with a free public education program titled "Your Personal Health Information: How to Access, Manage, and Protect It." The hour-long program provides the information you need to better manage your personal health information. Topics include how to access your medical records, your privacy rights, and starting a personal health record (PHR).

We will provide the speaker and audience handouts, at no cost to you. In return we ask that you provide the audience, a suitable date, time, and location for the presentation, and any technical or logistical support for the presentation (i.e. – laptop, projector, audio speakers, microphone, refreshments for attendees).
